Name alterations require the name to appear in the Birth Certificate, certified by Consular Division, MFA Colombo. Female applicants adding a husband’s name must also provide marriage documents.
- Certified copy of Birth Certificate (certified by Consular Division, MFA Colombo)
- For female applicants: original or certified copy of Marriage Certificate (certified by MFA Colombo)
- Passports of both husband and wife with two photocopies of data pages
- Completed Form “O”
Inclusion / Change of Profession

- Completed Form “O”
- Profession confirmation certificate from employer, endorsed by respective Chamber in Saudi Arabia
- If certificate is from Sri Lanka: certified by Consular Division, MFA Colombo
